Health insurance claims:

The care facility should file directly with the insurer if they are in-network. If you are having issues, please contact the toll free number located on the back of your card. If that doesn't work, contact us and we will help you resolve any issue.

Disability insurance claims:

If the unfortunate should happen and you become disabled and can not return to work, locate your policy and determine if you have met the required waiting periods. If you do meet the requirements, you will need supporting documentation from your doctor that states how you became disabled, diagnosis code, description of how it happened, the dates of your disability and proof of your annual income. Once you have those items, contact us and we can assist you in filing the claim.
